You'll bounce from world content to boss arenas to the Pit, and by the time you're done, you've basically proved you can survive the season's worst habits: burst damage, puddles, and surprise mechanics.
Getting the Four Drops
Here's the clean order most players follow. 1) Purified Tongue of Azmodan drops from Azmodan during his World Boss appearance in Hawezar. Show up, tag, don't face-tank like it's a campaign boss. 2) Purified Claw of Andariel comes from Andariel in the Under City, where poison pressure can erase sloppy builds fast. 3) Purified Eye of Belial is tied to a Pit encounter with Belial; the run itself is the real cost, so bring a setup that can handle tight rooms and sudden spikes. 4) Purified Horn of Duriel is the weird one, because it's linked to a Hell Flood where Duriel can replace the Blood Maiden. People miss it all the time, then swear it's bugged.
The Altars and the "???" Portal
Once you've got all four, you're not done. You've got to physically visit four old altars spread across Hawezar, Nahantu, Fractured Peaks, and Kehjistan, and sacrifice each essence. Order doesn't matter, but the travel time does, so plan your route. After the last offering, a portal shows up labeled with question marks, and that's your invite. If your build's been skating by on "good enough," this is where it stops being funny.
Why It's Worth the Pain
The fight behind that portal hits hard, and it's the kind of hard that punishes lazy defenses—uncapped resists, low armor, no plan for sustain. But the reward is real: a Majestic Spark for Mythic crafting, plus extra goodies like Under City Tributes and cosmetics. If you're close but not quite there, lots of players tighten their setup first, sometimes even choosing to buy diablo 4 gear so the whole essence grind doesn't end in one ugly one-shot.
